Job Description:

SUMMARY:

This position assesses the needs and determines initial and ongoing eligibility of clients for programs including, but not limited to, Food Assistance, Medicaid, Adult Financial, Colorado Works, and the Colorado Child Care Assistance programs for Prowers County Human Services department; provides case management services, and assists clients in accessing community resources. Duties may vary according to job assignment.

Essential duties and responsibilities may include, but are not limited to, the following:

  • Assesses economically vulnerable clients; evaluates clients' needs, provides comprehensive eligibility determinations to families or cases in accordance with agency, state, and federal program requirements.
  • Conducts one-on-one interviews with applicants, recipients, and other related parties to obtain information required to provide client services and/or determine program eligibility.
  • Interprets and applies complex federal and state policy to ensure access to appropriate services and consistent, accurate, and timely processing of client services.
  • Maintains thorough understanding of complex programs, processes, or specialized systems through participation in ongoing training and self-teaching.
  • Screens clients' eligibility for community services, searches for community partners and service providers to assist clients, and refers client to community agencies based on needs and problems identified in the assessment; coordinates multiple services as required.
  • Operates databases from a wide variety of systems, provides program services, ongoing case maintenance, and/or to processes applications for assistance and redeterminations of eligibility.
  • Accurately enters information into and/or receives information from the various computer eligibility systems (CBMS, EBT, SAVE, CUBS, and CSE).
  • Updates and maintains a variety of files, records, and other documents; gathers, compiles, and synthesizes data for reporting purposes; develops summary records and prepares reports, including court reports; prepares case files and assessments for review.
  • Handles confidential and sensitive information on a regular basis.
  • Attends scheduled department meetings and trainings as required.
  • Participates in external stake-holder meetings and community events to promote services.
  • Supports the relationship between Prowers County government and the general public by demonstrating courteous and cooperative behavior when interacting with citizens, visitors, and County staff; maintains confidentiality of work-related issues and County information; performs other duties as required or assigned.
  • Maintains regular and reliable attendance.
  • Ensures accuracy of applicant/recipient information and conducts eligibility determinations within established guidelines to meet all state and County goals and avoid sanctions.
  • Determines the need for recovery of funds or referral for fraud investigations. Completes informational reports to the proper investigative resource in cases of suspicioned fraud. Verifies pertinent statements given the agency by phone contacts, written requests, and/or direct contact with collateral resources Prepares for and testifies at evidentiary or other hearings.

MINIMUM QUALIFICATIONS:

Education and Experience:

  • High School Diploma or GED is required
  • Associates degree in a human services or business management field and/or two years of full-time responsible office clerical or public contact experience will be considered.

KNOWLEDGE AND ABILITIES:

Knowledge of:

  • Specialized interviewing and communication techniques.
  • Underlying social and physical factors facing client groups, and strategies to resolve core issues.
  • Case management methods, practices, and procedures for assigned client population.
  • Local community issues and regional community resources available to citizens.
  • County and Department specific policies

Ability to:

  • Establish and maintain effective working relationships with other County employees, representatives of other agencies and organizations, and members of the community.
  • Provide professional customer service to clients and public  .
  • Perform data entry, typing, and computer work at sufficient level of skill to accurately complete a relatively heavy workload in a timely manner.
  • Handling multiple tasks simultaneously, under pressure, and in emergency and stressful situations.
  • Following and effectively communicating verbal and written instructions.
  • Utilizing standard personal computer software programs and specialized state and County software.
  • Communicating effectively verbally and in writing.
  • Accomplish the assigned workload in a timely manner and meet established state and County performance standards, objectives, and deadlines.
  • Perform basic mathematical calculations including addition, subtraction, multiplication, and division.

WORK ENVIROMENT:

Position works in a standard office environment with 5% of time spent performing job-related duties outside of the office.

PHYSICAL DEMANDS:

The following are some of the physical demands commonly associated with this position.

  • VISION - Individual must be able to read handwritten or printed material.
  • HEARING - Individual must possess average hearing ability.  The noise level in the work environment is usually moderate.
  • LIFTING - Must be able to lift, carry, push or pull a minimum of 45 pounds. 
  • MOBILITY - Spends 40% of the time sitting, 40% standing, and 20% walking while at work, although this may vary.  Occasionally stoops, kneels, balances, reaches, crawls, and crouches while performing manual duties.
  • DEXTERITY - Frequent use of eye, hand, and finger coordination enabling the use of automated machinery.
  • COMMUNICATION - Individual must be able to clearly understand the English language and be understood while speaking the English language.
  • DRIVING - Individual must be able to operate a motor vehicle.
